What Is an OASIS Assessment? OASIS-E Explained for Home Health
OASIS-E2 explained for home health clinicians: what OASIS is, current versions, timepoints and deadlines, how long it takes, and faster SOC narratives.
OASIS is the Outcome and Assessment Information Set, the standardized patient assessment Medicare-certified home health agencies complete at the start of care and at set points after it.1 Since July 1, 2025 it is collected for patients of every payer, not only Medicare and Medicaid.2 The answers feed quality measures and payment.
OASIS is not the whole assessment. It is a set of standard data items built into your agency’s own comprehensive assessment, and some patients are exempt from it entirely.3 The version family in use is OASIS-E, currently OASIS-E2.
Which version is current
Three dates, and they matter, because a lot of what turns up in a search is out of date.
OASIS-E took effect January 1, 2023. OASIS-E1 followed on January 1, 2025. OASIS-E2 took effect April 1, 2026, and the OASIS-E2 guidance manual is the one in use now.4 If you have a manual saved on your desktop, check the date on it.
What is OASIS-E?
OASIS-E is the version of the OASIS data set CMS introduced on January 1, 2023. It replaced OASIS-D1 and lined up home health assessment items with other post-acute settings. It has been refined twice since, by OASIS-E1 in January 2025 and OASIS-E2 in April 2026, which is current.
One more change worth knowing. Since July 1, 2025, OASIS is required for any patient receiving skilled services who is not exempt, whoever pays. Charity care, self-pay, no payer at all. The trigger is a start of care M0090 date on or after that day.5
Timepoints and deadlines
First, a distinction that trips people up. The initial assessment visit is not an OASIS timepoint. It is a separate requirement under the Conditions of Participation, due within 48 hours of referral, within 48 hours of the patient’s return home, or on the ordered start of care date.6 OASIS is collected with the comprehensive assessment, which runs on its own clock.
Here is the OASIS schedule. SOC is start of care, ROC is resumption of care, and RFA is the reason for assessment you code at M0100.7
| Timepoint | RFA | Assessment timeframe |
|---|---|---|
| Start of Care | 1 | Within 5 calendar days after the SOC date, SOC is day 0 |
| Resumption of Care | 3 | Within 2 calendar days of the facility discharge date or knowledge of the return home |
| Recertification | 4 | The last 5 days of every 60 days, days 56 to 60 |
| Other Follow-up | 5 | Within 2 calendar days of a significant change in condition |
| Transfer, not discharged | 6 | Within 2 calendar days |
| Transfer, discharged | 7 | Within 2 calendar days |
| Death at Home | 8 | Within 2 calendar days |
| Discharge | 9 | Within 2 calendar days |
Resumption of care gets asked about more than any other timepoint, so two plain sentences on it. A qualifying inpatient stay is 24 hours or longer for a reason other than diagnostic testing. If the practitioner ordered a specific resumption date, that date starts the clock instead of the hospital discharge.8
Two more. Observation stays and time in the emergency department do not meet the definition of a transfer, however long they run.9 And the recert window is counted from the original start of care date, not from a resumption of care date, so a hospitalization mid-episode does not reset it.10
How long does an OASIS take
No official answer exists. The rules set deadlines, not durations. CMS tells you when the assessment is due and never how long you get to spend on it.
What clinicians describe is about an hour or more for the documentation of a start-of-care visit, and longer for complicated admissions. Several say the number depends less on the patient than on the agency’s own documentation standards, which vary a lot from one place to the next.
Two things in the guidance change the math, and plenty of clinicians have never been told either one.
The assessment does not have to be finished in one sitting in the home. M0090, the date assessment completed, is the last date you gathered information and finished documenting it. It does not have to be the date of a visit. If you call the family or the physician two days later to close out an item you could not answer in the home, M0090 reflects that date even though no visit happened.11
You are also allowed help. The assessing clinician owns the assessment and signs it, but you can take input from the patient, the caregiver, the physician, the pharmacist, and other agency staff who have had contact with the patient.12 Collaboration has to happen inside the assessment timeframe, and your agency sets the policy for how it gets documented. Some agencies limit it to what one clinician collected directly. Worth knowing which yours does.
How long should an OASIS recert take?
No rule sets a length for a home health recertification. The rule sets the window, which is the last 5 days of every 60 days. How long it takes depends on the patient, on which items apply at that timepoint, and on what your agency wants in the narrative.

How experienced clinicians speed it up
A start-of-care narrative covers the same ground every time. Reason for referral. Homebound status. Findings by system. Skilled need. Plan.
The structure repeats. What goes inside it does not, and that distinction is the whole game.
CMS does not prohibit templates. The Program Integrity Manual says reviewers consider information captured in templates, and that the ones CMS discourages are the ones with too little room, the check boxes and predefined answers and cramped fields that fail to capture enough clinical detail. It also warns that a template built mainly for billing usually does not show medical necessity.13
Homebound wording carries its own caution. The Medicare Benefit Policy Manual says the determination rests on the patient’s overall health status, the diagnosis, the functional limitations, the clinical course, and not on standardized phrases alone.14 A considerable and taxing effort sentence with nothing patient-specific in it is the sentence a reviewer stops on.
How do I write an OASIS SOC narrative faster?
Keep the structure saved as a template and type only what is specific to this patient. The parts that repeat are the section order, the lead-ins, and the frame of the homebound and skilled-need sentences. The evidence inside them changes at every admission.
Text Blaze keeps those parts one shortcut away, and it has a free plan. You type a short shortcut in the field you already write in. A pop-up shows up with placeholders for this patient’s details, such as the vitals, the diagnosis and what you found. You click insert and the full finished text appears in your own words. You save a few minutes of typing every time. Your typing goes to the findings and the evidence, the part only you can write, and the frame you used to retype at every admission is already there.
That frame is captured in the OASIS-E Cheat Sheet & Phrase Pack. Start-of-care and recert narratives, homebound and skilled-need lines, and an F2F attestation block, installed in one click and yours to edit. Use /socnar at your next admission, then save one sentence of your own. The pack’s homebound sentence, /taxing, leaves the reason blank on purpose, because the reason is the part a reviewer is looking for.

Common questions
What is OASIS documentation?
The completed OASIS items for a timepoint plus the clinical documentation supporting them, including the visit note and the narrative. The two have to agree. Reviewers compare your OASIS answers against the rest of the record, and a mismatch is a data quality problem whether or not the care was fine.
Does Medicaid require OASIS?
Yes, for Medicaid home health providers in states whose laws require those agencies to meet the Medicare Conditions of Participation.15 Since July 2025 the question matters less than it did, because OASIS now applies to any patient receiving skilled services who is not exempt, whoever the payer is.
Who can complete an OASIS assessment?
A registered nurse, physical therapist, speech-language pathologist, or occupational therapist. An LPN, LVN, PTA, COTA, social worker, or home health aide may not. When nursing and therapy are both ordered at referral, the RN completes the start of care.16 You can collaborate with other staff, but one clinician owns the assessment and signs it.
Where is the OASIS-E guidance manual?
On the CMS OASIS User Manuals page. The current one is the OASIS-E2 guidance manual, effective April 1, 2026, posted alongside the OASIS-E2 questions and answers.17
Is there OASIS-E training or certification?
CMS offers free training through its Home Health Quality Reporting Training page, including a Home Health QRP Essentials course covering the program and its reporting requirements.18 Private certifications exist and some agencies pay for them, but CMS requires none of them to complete an OASIS.
