About this snippets template

A QuickDASH calculator and five more outcome measure calculators for PT notes. Type a shortcut where you document, enter each answer from the official form the patient completed, and the score, the band and the date land in your note as one result line. The math is done for you, and you check every result.

What's inside the QuickDASH calculator pack

Score calculators:

  • /qdash: QuickDASH, 11 answers, scored 0 to 100 when at least 10 are answered
  • /odi: an Oswestry disability index calculator, percent over the sections answered, with the disability band
  • /ndi: a neck disability index calculator, total out of 50 and percent, with the band
  • /lefs: lower extremity functional scale scoring, total out of 80 and percent of maximal function
  • /dhi: dizziness handicap inventory scoring, total out of 100 with functional, emotional and physical subscales
  • /psfs: the full Patient-Specific Functional Scale, up to five activities the patient names, with the average

Shorter snippets:

  • /omres the result line with today's score and the previous one
  • /mcid a sentence noting the change exceeds the minimal clinically important difference

How the calculators work

  • The QuickDASH, ODI, NDI, LEFS and DHI calculators label each item with a short topic. Patients still fill in the official form, and each calculator names its developer and where to get that form
  • Pick each answer from a drop-down menu. A switch can add the item scores to the note if your clinic records them
  • Type the shortcut in WebPT, Prompt, Net Health or any browser EMR, or in installed programs through the desktop app
  • Check every score and band against the developer's scoring guide

Free to install. Use /qdash on your next upper limb patient. Then save one sentence you type often as your own snippet: /omres is a good first one, and it puts today's score next to the last one in a few seconds.
